Saint Joseph PACE (Programs of All-inclusive Care for the Elderly) program provides care and services to seniors with chronic care needs allowing them to maintain their independence in their homes for as long as possible. The program offers a full range of health care services for individuals who are eligible for nursing home care but want to live at home. Saint Joseph PACE believes it is better for seniors with chronic care needs to be served in their home whenever possible. Many PACE participants get most of their care from staff employed by the PACE organization in the PACE center. Each PACE participant receives customized care that is planned and delivered by a coordinated, interdisciplinary team of professionals working at the center. The team meets regularly with each participant and his or her representative in order to assess the participant's needs. A participant's care plan usually integrates some home care services from the team with several visits each week to the PACE center, which serves as the hub for medical care, rehabilitation, social activities and dining.

If your health care team decides you need care and services that Medicare and Medicaid doesn't cover, PACE may still cover them. If you have Medicaid, you won’t have to pay a monthly premium for the long‑term care portion of the PACE benefit. If you have Medicare but not Medicaid, you’ll be charged a monthly premium to cover the long‑term care portion of the PACE benefit and a premium for Medicare drug coverage . However, in PACE, there’s never a deductible or copayment for any drug, service, or care approved by the PACE team of health care professionals. Founded as Boulder Hospice in 1976, TRU is a Colorado-licensed, Medicare and Medicaid-certified, nonprofit health care organization serving Boulder, Broomfield, Adams, Jefferson, Arapahoe, Denver, and Weld Counties and beyond. Focusing on a single aspect of care does not provide the older person with the best chance of remaining independent in the community.

TRU PACE provides initial assessments for all participants in order to determine which services are needed and where they will best be served. The potential organization submits the formal application to CMS with the ALTSA assurance that the entity is considered to be qualified as a PACE organization and is willing to enter into a PACE organization agreement with the entity. The PACE patient community is very diverse, made of individuals from different backgrounds and with a variety of health issues. PACE provides its participants with all services covered by Medicare and Medicaid, without the limitations normally imposed by these programs. It also provides any other services deemed necessary by the interdisciplinary team that would allow program participants to remain in the community. This site provides day center services for the elderly and a health center to the most participants of any site. It is also home to PACE-RI’s administrative, operational, and clinical staff. PACE provides all the care and services covered by Medicare and Medicaid if authorized by your health care team.
